Export of Disclosure Forms has been updated to export all date fields in the mm-dd-yyyy format. This report is accessible in the portal from the View Disclosure Forms page (Menu: Disclosure Forms -> View), apply desired search criteria in the filters and select the “Export Disclosure Form Data” button at the top of the page.
